and post-hospital care services in a caring, supportive environment.

The focus of the program is to help each
client realize their optimal level of functioning, good health,
independence and well-being. This is accomplished by a well-coordinated
interdisciplinary team of professionals who assess each client
and develop an individualized plan of care for them. A network
of program components is organized by the Program Director to
provide a continuum of care for each client consistent with their
individual needs. |

the greater Newburyport, Merrimack Valley and the North Shore
areas of Massachusetts as well as the bordering New Hampshire
communities.

The Center is open daily Monday
through Friday
from 8:00 a.m. to 4:00 p.m. and is
located at 166 High Street, P.O. Box 117
in Newburyport, Massachusetts.
Participants' attendance schedule
can be individualized according to need.
