The focus of the program is to help each client realize their optimal level of functioning, good health, independence and well-being. This is accomplished by a well-coordinated interdisciplinary team of professionals who assess each client and develop an individualized plan of care for them. A network of program components is organized by the Program Director to provide a continuum of care for each client consistent with their individual needs.

 

 

Harborside Adult Day Health Center serves the greater Newburyport, Merrimack Valley and the North Shore areas of Massachusetts as well as the bordering New Hampshire communities.

from 7:30 a.m. to 3:30 p.m. and is
located at 166 High Street, P.O. Box 117
in Newburyport, Massachusetts.
